Thank you for your interest in becoming an egg donor! The Fertility Institute of Hawaii is seeking healthy women of all ethnicities, between the ages of 21-28 years old. Applicants must be non-smokers, drug and disease-free, college-educated or have a high school diploma/GED, able to access family medical history (on both sides), and committed to all screening appointments. 

The application process consists of two phases:

1) The Prescreen Questionnaire (complete below): This questionnaire is a short assessment that checks if you meet the basic qualifications to become an egg donor. If you meet the requirements, you will be directed to complete the full questionnaire in the next step.

2) Full Questionnaire: A slightly more detailed survey that dives into more detail on everyone's favorite topic - YOU! So, to begin, take a moment to sit back, relax, and complete the pre-screener questionnaire below:
